Each Scout is required to have an annual physical on file with his troop. All physicals must include your shot record and a copy of your parents insurance card, front and back. This form is a fillable PDF that can be saved and updated each year. It is strongly recommended that you take advantage of this feature.

Scouts who require medication will need to complete the prescription dosing form to accompany the medications in their original containers. As a matter of convenience, please place this completed form along with the listed medications in a zip-top bag marked clearly with the Scout's name.
